RN-Care Manager

The Parker Jewish Institute, located in New Hyde Park, NY, is seeking an RN-Care Manager to join its team of professionals at Parker Jewish Institute's Certified Home Health Agency. 


The Parker Jewish Institute for Health Care and Rehabilitation, conveniently located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility that offers short term rehabilitation, sub-acute care and nursing home care, as well as community-based health care' encompassing adult day health care, home health care, and a hospice program that serves terminally ill patients in their own homes or in nursing facilities, including Parker's nursing home.

Parker Jewish Institute is seeking a Full Time RN-Care Manager. The Care Manager is responsible for the overall care management and quality of care for members and is responsible for performing and reviewing assessments, coordinating plans of care and educating the members on proper health management


  • May perform in home assessments as part of the care management to create and coordinate the patient specific plan of care
  • Knowledgeable of the rules and regulations guiding the member per their insurance qualifications
  • Coordinates benefits and arranges services for members of AgeWell New York
  • Lead and facilitate IDT activities and communication; facilitate assessments of participant needs; assist in developing, implementing and monitoring the member's care plan
  • Appropriately documents any new findings in the member's care management record


  • Clean and active NYS RN license
  • Computer literate in Care Management EMR
  • Bilingual in Spanish highly preferred
  • Care Management certification preferred (CCM)
  • Prior case management experience
  • At least one (1) year prior experience in a community based healthcare setting
