The Parker Jewish Institute, located in New Hyde Park, NY, is seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our team of professionals in the Nursing Department on the Evening Shift.

 LPN's at Parker Jewish Institute for Health Care and Rehabilitation are responsible for performing skilled resident care on under the supervising of an RN. LPN's may supervise C.N.A's.

Key Tasks:

  • Collects patient/resident data to facilitate completion of clinical assessments
  • Participate in problem identification
  • Follows CCP and care priorities
  • Collaborate with other health professionals in the Plan of Care
  • Monitors patient progress and condition and trends requiring intervention
  • Add pertinent information to the plan of care as needed
  • Observe patient/resident response to therapeutic regimen and report to RN.
  • Administer medications and treatments and assist with diagnostic tests and procedures
  • Participate in resident/family education and discharge planning
  • When in charge, manage work assignments to meet patient and unit needs
  • Supervise and monitor delivery of care by Nursing Assistants
  • Participate in orientation of new staff or students on unit
  • Maintain notes and records of patient status, and transcribe physician orders

 

Qualifications:

  • Current and valid NYS License of Practical Nursing (LPN)
  • Graduate of an accredited Nursing school
  • Recent, relevant experience working with the geriatric population
