RN-Community Health and Wellness Liaison

Parker Jewish Institute in New Hyde Park, NY is seeking a Part Time RN- Community Health and Wellness Liaison to join our team of professionals in the Outreach department.

Position Overview

  • Conducts outreach assessments
  • Uses assessments to identify high risk clients and coordinate their health and long-term care
  • Works with clients to build a caseload of health management clients who receive assistance with medication compliance, education about chronic illness, 30 safety assessment and home modification, coordination with other health care providers and family counseling plan for current and future needs
  • Links members of the community with medical, home health, hospice and rehabilitation services
  • Works closely with hospital and home health agency staff to ensure that individuals are not discharged without the appropriate service coordination
  • Carries out health promotion activities based on results of community assessment in collaboration with Parker Jewish Institute and other health care providers
  • Participates in regular team meetings with NORC site and Parker Jewish Institute when required and requested

 

Key Tasks:

  • Health care assistance and health care management for NORC members involving: Blood pressure monitoring, medication review, glucose monitoring, doctor follow-up, family consults
  • In-office and home visitations
  • Workshops on topics of need
  • Medical support groups-i.e: diabetes, cancer support
  • Referrals to appropriate home care agency, MLTC or FIDA
  • Follow-up on the Health Indicators Project as mandated by DFTA

 Position Requirements

  • Active RN license in New York State
  • Prior experience in a community or outreach position
  • Prior case management experience preferred
